How do I add an executor or representative's name to a deceased taxpayer's refund check?

In order for the Department of Taxation to reissue a refund check to include the name of an executor or representative, you must provide all of the following:

  • The original refund check;
  • A copy of the taxpayer’s death certificate; AND
  • Federal form 1310.

Note: Instead of adding the executor or representative’s name to the refund check, the Department can reissue the refund check with the word “deceased” next to the decedent’s name. In this case, you are not required to submit federal form 1310.
