How do I take the identity confirmation quiz?

You can only take an identity confirmation quiz if you received a quiz letter from the Department of Taxation. You will have 30 days from the date of your letter to take the quiz. Processing of your return does not begin until your quiz is passed. To take the identity confirmation quiz:

  1. Go to the Department’s Identity Confirmation page;
  2. Select the following option:Green Checkbox
  3. Check the “I Agree” box (see below) and enter the Reference Number and Authorization Code shown on your letter;
  4. Enter your “State of Ohio Refund Dollar Amount” as a whole number (no decimal places);
  5. Correctly respond to the Verification Question;
  6. Click the Next button at the lower right portion of the screen.

Note: By checking the “I Agree” box, you authorize the Ohio Department of Taxation, under the FCRA (Fair Credit Reporting Act), to obtain information from your personal credit profile for the sole purpose of confirming your identity. See 15 U.S.C. §1681. Taking the identity confirmation quiz has no effect on your credit score.

Once you have accessed the quiz, you will have 5 minutes to answer four multiple choice questions. If technical difficulties prevent you from completing the quiz in the time allotted, it will count as a failed attempt.

After you have selected an answer for all four questions, click the Submit button. You must answer three of the four questions correctly to pass the quiz.
