The Ohio Department of Taxation has compiled a list of frequently asked questions covering many different categories.

I received a letter about the Ohio Adoption Credit I claimed in tax year 2014 (and forward). What information do I need to submit to verify that I am entitled to the credit?

Court papers which provide evidence of the legal adoption of a minor child under the age of 18.

AND

Receipts and proof of payments (cancelled checks, bank expenses, and or credit card statements) for expenses incurred in the adoption process. Federal Form 8839 is not acceptable documentation as proof for expenses incurred.
