Frequently Asked Questions

The Ohio Department of Taxation has compiled a list of frequently asked questions covering many different categories.

To view the questions, click on the "Select Category" bar and then click on the category you are interested in.  A list of questions will appear pertaining to that category. Then click on the question you are inquiring about and the answer will appear.

I received an Ohio identity confirmation quiz letter, but I DID NOT file an income tax return with the state of Ohio. What should I do?

If you received an Ohio identity confirmation quiz letter, this indicates that an Ohio income tax return was filed in your name and a refund is requested on this return. If you did not file the return, please follow the instructions listed on the letter in order to report the return as suspect. After identifying the return as suspect, we encourage you to review other Identity Theft FAQ’s.

NOTE: Identifying the return as suspect will aid in the processing of your actual return when filed, if you have an Ohio filing requirement. If you have a preparer or someone other than yourself submit returns on your behalf, it is recommended that you check with them prior to reporting a return as suspect.
