If you are filing an Ohio income tax return for the first time or if you are unable to register, you will be required to fax the following information:
1) Send a clear copy (lighten and enlarge) of your Social Security Card (if you do not have a social security number, you must obtain a taxpayer identification number from the Internal Revenue Service)
2) You will also need at least one of the following:
a) Your current, valid driver's license or state issued identification card
b) U.S. passport
c) Military ID - both front and back sides are required
Include your date of birth AND daytime phone number or email address in the event we need to contact you.
IF YOU ARE ATTEMPTING TO FILE A JOINT RETURN, PLEASE INCLUDE THE DOCUMENTATION REQUESTED ABOVE FOR BOTH PARTIES.
