Frequently Asked Questions

The Ohio Department of Taxation has compiled a list of frequently asked questions covering many different categories.

To view the questions, click on the "Select Category" bar and then click on the category you are interested in.  A list of questions will appear pertaining to that category. Then click on the question you are inquiring about and the answer will appear.

How do I know if my tax records have been affected?

Usually an identity thief uses a legitimate taxpayer’s identity to illegally file a tax return and claim a fraudulent refund. Generally, the identity thief will use a stolen SSN to file a forged tax return and attempt to get a fraudulent refund early in the filing season.

You may not be aware that this has happened until you file your return later in the filing season and discover that another return has already been filed using the same SSN.

Be alert to possible identity theft if any of the following is applicable to you:

  • You receive a notice from the IRS and/or Ohio Department of Taxation that more than one tax return for you was filed or
  • You receive a balance due notice, a refund offset notice, collection actions taken against you and/or a request for supporting documentation for a tax year  for which you did not file a personal income tax return or
  • Upon attempting to electronically file your personal income tax return, you receive a response that a return has already been filed for that SSN or
  • You receive a notice from the IRS and/or Ohio Department of Taxation indicating that you may be a victim of identity theft.
