Forms and information are provided by municipalities, at links in the table below. If listed as "unavailable," or if the site at the municipal link does not provide sufficient information, the City of Columbus Income Tax Web page contains tax rates and points of contact for all Ohio municipalities. Please visit Columbus Income Tax - Tax Municipalities and scroll the list to obtain rate and contact information for the municipality you seek.

If you're unsure of what municipal tax you're liable for, you can use the online tool The Finder to look up your address to verify what municipality you live in and its tax rate.
