The Ohio Department of Taxation is providing a location on its Internet Web site from which anyone can contact the cities and villages in Ohio with a municipal income tax and obtain forms and information (please see table below).
Forms and information are provided by municipalities, at links in the table below. If your municipality is not listed below, there is no Web site available. Also, some Web pages may be the municipality's main page, or may contain incomplete information. If you cannot locate your municipality or find sufficient information about it, please check the City of Columbus Income Tax Web page and select "Tax Municipalities," which lists the tax rates and points of contact for all Ohio municipalities.
If you're unsure of what municipal tax you're liable for, you can use the online tool The Finder to look up your address to verify what municipality you live in and its tax rate.
