Fax on Demand

Ohio is now offering "Fax on Demand." Listed below are the forms currently available. Taxpayers may call 1-800-282-1782 to have tax forms mailed or faxed to them. Customers will be prompted by an automated attendant who will tell them to press:

• If you would like to have a transcript of your previous year's Individual Income Tax return, press 1
• If you would like to have a form faxed, press 2
• If you would like to have a form mailed, press 3
• If you have a bulk order, press 4 or
• To repeat these options, press zero

Callers should then select the tax type and the attendant will list the available forms.

Taxpayers will receive fax requests within 24 hours, and mail requests within three business days.
