Research: ODT Request for Data

To All County, Municipal and Township Governments,

On an annual basis, the Ohio Department of Taxation requests completion of surveys on many types of taxes including property tax, income tax (municipal), lodging tax and local government funds distribution. These forms are mailed via regular US post, but are also made available here for ease and efficiency.

Please review the following list for the survey or certification you’re in need of; each survey has instructions and information listed. Please refer to these if questions arise, or contact Rose_Vidmar@tax.state.oh.us.
